Private Music Lesson Enrollment Form

Please use the form below for enrolling in Private Music Lessons.
You may enroll multiple students (additional family members) on one form by clicking the ‘add another student’ button after submitting initial info.
Note: When submitting this form, you will be charged a, one-time, $5 registration fee, and your card will be saved on file in our system for tuition charges.
- When you start lessons, this fee will credited into your account & applied towards your first tuition invoice.
- This registration fee is non-refundable & will also hold your spot on our Waiting List in the rare case that we’re not able to find a lesson time for you right away.
Be sure you complete all three form pages, which should be as follows: Student Info —> Contact Info —> Payment Info & Submission.
- After complete all three, you’ll get a confirmation from us & we’ll reach out soon with available lesson times.
- Some browsers can scroll to the bottom section of our website between form pages. If that happens, simply scroll back up & you should see the next page on the form.
